Which Forms Are Required For New Patients?

When you arrive for your first visit, please be prepared to complete all insurance and health information forms so that we can begin your dental treatment. We will ask you to fill out several forms, such as a patient registration form and medical history form.

What Does The Dental Exam Include?

The first visit helps our team become acquainted with each patient and learn about their goals and desires regarding dental treatment. This 90-minute examination does not include a teeth cleaning, but prior arrangements can be made to accommodate this, should additional care be needed. Oral hygiene instructions will also be provided along with suggestions to help the patient care for their teeth. Our comprehensive dental examination includes the following.

  • Checking muscles & joints that support the mouth
  • Oral cancer screening
  • X-rays specific to your needs.
  • Inspecting joint function and tooth arrangement
  • Dental charting
  • Assessing gum health and bone health (Periodontal Health Screening)
  • Intra-oral pictures of the teeth & supporting structures
